Psychosis refers to a serious mental disorder (such as schizophrenia) characterized by defective or lost contact with reality, often with hallucinations or delusions.

Rationale
Practice guidelines for the treatment of schizophrenia recommend that patients in a stable phase can be given a trial off antipsychotic medications if they have had only one episode with at least one subsequent year free of positive symptoms, or multiple episodes and 5 years without positive symptoms. During the drug free period, clinical management should include a plan to monitor and respond to early signs of relapse.
