Risk Assessment for Self Harm Overall Rank: 2 Best in Domain
Healthcare professionals attending a person who has self-harmed should conduct and record a comprehensive and respectful assessment of (in order of urgency):
1. risk
2. current emotional and mental state
3. psychosocial needs
4. main clinical and demographic factors associated with risk of further self-harm and/or suicide.
This special area domain refers to those patients whose conditions require an immediate intervention, such as an emergency room visit.

Rationale
Everyone who has self-harmed should have a comprehensive assessment of needs and risk; engaging the service user is a prerequisite.

  • Especially important with high rate of suicides in First Nations communities
  • Most of these assessments should be done in normal PHC sites, by organizing schedules of PHC staff to allow for after hours home/office visits
