Long-term independent living program (7 days a week, team availability; available 8am - 8pm daily) where a team works with adults aged 25-64 years with serious mental illness compared to case management model where caseloads and patients are seldom seen more often than every 3 weeks. Desired outcome is reduced hospitalization; timely intervention for recurrence; and emphasis on practical assistance related closely to quality of life enhancement.

  • Support staff should be available but not neccesarily living with them.
  • Highly relevant for First Nations.
  • Available to those who need them i.e. unable to live independently in the community without this service
  • I disagree with others comments that this is not PHC relevant, it is a key issue in the stable/maintenance phase of illness care
